Good Faith Estimate
Good Faith Estimate Notice
Under the federal No Surprises Act, health care providers must provide an estimate of the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created and includes a projected cost of services over time for clients who are uninsured or who choose not to use insurance to cover the cost of health services.
You have the right to receive a Good Faith Estimate explaining the total expected cost of services before scheduling or upon request. The Good Faith Estimate is not a contract or a bill.
To request a Good Faith Estimate, please contact Sapphire Blooms Counseling PLLC at info@sapphirebloomscounseling.com.
